1. Sale of Tickets

  1. Purchaser must be 18 years or older to play and physically in Ontario at the time of purchase.
  2. Tickets will be sold using an Alcohol and Gaming Commission of Ontario (AGCO) approved electronic raffle system supplied by Ascend Fundraising Solutions. Tickets can be purchased online at www.MHF5050.ca using a credit card as form of payment. Accepted credit cards are VISA, Mastercard and American Express.
  3. Each request for a refund will be assessed on a case-by-case basis. If the request for a refund has merit and is made prior to the draw, Mackenzie Health Foundation will determine if a refund is eligible.
  4. After buying, patrons will receive a confirmation e-mailed with their 50/50 registered numbers from a Random Selection System. No specific numbers can be given as numbers are generated randomly.

2. Draw Schedule and Results

  1. The online Lottery draws will begin on November 1, 2021 and continue on a monthly basis until March 25, 2022. All times are Eastern Standard Time (EST).
    • Draw No. 1: Tickets sales will begin November 1, 2021 at 11:00 a.m. and end November 29, 2021 at 11:59 p.m. The Grand Prize winner will be drawn on November 30, 2021 at 11:00 a.m. This draw will have a guaranteed Grand Prize of $5,000. When the ticket sales exceed $10,000, the winning prize will be half of the sales.
      • Tickets are $50 for 150, $20 for 30, and $10 for 5
      • There will be (1) Early Bird $1,000 cash prize awarded and tickets purchased between November 1, 2021 at 11:00 a.m. to November 15, 2021 at 11:59 p.m. will be eligible. The Early Bird winner will be drawn on November 16, 2021 at 11:00 a.m.
    • Draw No. 2: Tickets sales will begin December 1, 2021 at 11:00 a.m. and end December 29, 2021 at 11:59 p.m. The Grand Prize will be drawn on December 30, 2021 at 11:00 a.m. This draw will have a guaranteed Grand Prize of $5,000. When the ticket sales exceed $10,000, the winning prize will be half of the sales.
      • Tickets are $50 for 150, $20 for 30, and $10 for 5
      • There will be (1) Early Bird $1,000 cash prize awarded and tickets purchased between December 1, 2021 at 11:00 a.m. to December 14, 2021 at 11:59 p.m. will be eligible. The Early Bird winner will be drawn on December 15, 2021 at 11:00 a.m.
    • Draw No. 3: Tickets sales will begin December 30, 2021 at 12:00 p.m. and end January 27, 2022 at 11:59 p.m. The Grand Prize will be drawn on January 28, 2022 at 11:00 a.m. This draw will have a guaranteed Grand Prize of $5,000. When the ticket sales exceed $10,000, the winning prize will be half of the sales.
      • Tickets are $50 for 150, $20 for 30, and $10 for 5
      • There will be (1) Early Bird $1,000 cash prize awarded and tickets purchased between December 30, 2021 at 12:00 p.m. to January 13, 2022 at 11:59 p.m. will be eligible. The Early Bird winner will be drawn on January 14, 2022 at 11:00 a.m.
    • Draw No. 4: Tickets sales will begin January 28, 2022 at 12:00 p.m. and end February 24, 2022 at 11:59 p.m. The Grand Prize will be drawn on February 25, 2022 at 11:00 a.m. This draw will have a guaranteed Grand Prize of $5,000. When the ticket sales exceed $10,000, the winning prize will be half of the sales.
      • Tickets are $10 for 5, $20 for 30, and $50 for 150
      • Wine & Dine Wednesdays! #1: All tickets purchased between January 28, 2022 at 12:00 p.m. to Tuesday, February 1, 2022 at 11:59 p.m. will be entered into the Wine & Dine Wednesdays! #1 prize draw for a $250 gift card to Touro Brazilian Steakhouse, and the winner will be drawn on Wednesday, February 2, 2022 at 11:00 a.m.
      • Wine & Dine Wednesdays! #2: All tickets purchased between January 28, 2022 at 12:00 p.m. to Tuesday, February 8, 2022 at 11:59 p.m. will be entered into the Wine & Dine Wednesdays! #2 prize draw for a $250 gift card to Vivo Pizza+Pasta, and the winner will be drawn on Wednesday, February 9, 2022 at 11:00 a.m.
      • Early Bird: All tickets purchased between January 28, 2022 at 12:00 p.m. to Thursday, February 10, 2022 at 11:59 p.m. will be entered into the Early Bird prize draw for $1,000 cash, and the winner will be drawn on Friday, February 11, 2022 at 11:00 a.m.
      • Wine & Dine Wednesdays! #3: All tickets purchased between January 28, 2022 at 12:00 p.m. to Tuesday, February 15, 2022 at 11:59 p.m. will be entered into the Wine & Dine Wednesdays! #3 prize draw for a $250 gift card to Parola Ristorante, and the winner will be drawn on Wednesday, February 16, 2022 at 11:00 a.m.
      • Wine & Dine Wednesdays! #4: All tickets purchased between January 28, 2022 at 12:00 p.m. to Tuesday, February 22, 2022 at 11:59 p.m. will be entered into the Wine & Dine Wednesdays! #4 prize draw for a $250 gift card to Locale Restaurant, and the winner will be drawn on Wednesday, February 23, 2022 at 11:00 a.m.
    • Draw No. 5: Tickets sales will begin February 25, 2022 at 12:00 p.m. and end March 24, 2022 at 11:59 p.m. The Grand Prize will be drawn on March 25, 2022 at 11:00 a.m. This draw will have a guaranteed Grand Prize of $5,000. When the ticket sales exceed $10,000, the winning prize will be half of the sales.
      • Tickets are $10 for 5, $20 for 30, and $50 for 150
      • Early Bird #1: All tickets purchased between February 25, 2022 at 12:00 p.m. to Tuesday, March 1, 2022 at 11:59 p.m. will be entered to win a $250 cash prize, and the winner will be drawn on Wednesday, March 2, 2022 at 11:00 a.m.
      • Early Bird #2: All tickets purchased between February 25, 2022 at 12:00 p.m. to Tuesday, March 8, 2022 at 11:59 p.m. will be entered to win a $250 cash prize, and the winner will be drawn on Wednesday, March 9, 2022 at 11:00 a.m.
      • Early Bird #3: All tickets purchased between February 25, 2022 at 12:00 p.m. to Tuesday, March 10, 2022 at 11:59 p.m. will be entered to win a $1,000 cash prize, and the winner will be drawn on Wednesday, March 11, 2022 at 11:00 a.m.
      • Early Bird #4: All tickets purchased between February 25, 2022 at 12:00 p.m. to Tuesday, March 15, 2022 at 11:59 p.m. will be entered to win a $250 cash prize, and the winner will be drawn on Wednesday, March 16, 2022 at 11:00 a.m.
      • Early Bird #5: All tickets purchased between February 25, 2022 at 12:00 p.m. to Tuesday, March 22, 2022 at 11:59 p.m. will be entered to win a $250 cash prize, and the winner will be drawn on Wednesday, March 23, 2022 at 11:00 a.m.
  2. The Wine & Dine Wednesdays!, Early Bird and Grand Prize draws will be held online using an AGCO approved Random Number Generator. It is possible for the same ticket number to win any and all Wine & Dine Wednesdays! prizes, the Early Bird prize and the Grand Prize.
  3. The Wine & Dine Wednesdays!, Early Bird and Grand Prize draw winning tickets will be posted on www.MHF5050.ca. If information about the Winner is collected at the point of sale, the Winner will be contacted directly.
  4. Odds of winning the Prize are based on the number of Numbers sold.

3. Prize Claims and Prize Payment

  1. The winner may claim their prize by contacting Mackenzie Health Foundation at: 905- 883-2032, or by email at: 5050@mackenziehealth.ca.
  2. The winning ticket holder has 6 months to claim their prize from the date of the draw their ticket was purchased for.
  3. Upon presenting a winning 50/50 ticket, the Winner must:
    • Provide a photo ID proving their age to be over 18;
    • Completely fill out and sign the 50/50 Lottery Prize Claim Form issued by a Mackenzie Health Foundation staff member
    • Sign their name on the Winning Ticket
  4. Once the certified Winner is verified, and provides a photo ID, they will be paid by Electronic Fund Transfer (EFT) for the amount posted at the event for which they bought their ticket. Winners must have the original winning ticket and no copies. If the Winner cannot provide photo ID, then the winning proceeds will be held until such identification can be provided. EFT processing will commence after certification is finalized and processing times vary and can take up to two weeks.
  5. Each Winner acknowledges that Mackenzie Health Foundation, during the prize claim process, may require certain personal information from the Winner and that the collection of such information is necessary to the proper administration. Any personal information collected is intended to be used for following principal purposes: to comply with legal and audit requirements, for announcing winners, awarding prizes, and the posting of prize wins on Mackenzie Health Foundation’s website for an extended period of time. Each Winner further acknowledges that any failure by the Winner to provide Mackenzie Health Foundation with such information may prevent Mackenzie Health Foundation from paying or awarding all or part of the prize in the manner contemplated by the prize structure.
  6. Mackenzie Health Foundation will not and is not responsible for providing financial or tax advice to the Winner.
  7. For groups purchasing (1) ticket, group winners will select one individual to handle the single cash payment.
  8. All prizes must be claimed within six months of the relevant draw, unless otherwise specified by Mackenzie Health Foundation. Thereafter any unclaimed prizes shall be donated to a charity/non-profit organization approved by the Alcohol and Gaming Commission of Ontario (AGCO).
  9. Mackenzie Health Foundation will not award a prize for tickets which are void unless Mackenzie Health Foundation, in its discretion, deems it appropriate to do so. Tickets are void if lost, stolen, unissued, illegible, mutilated, damaged, altered, counterfeited or forged, miscut, misregistered, defective, misprinted, cancelled, produced in error and not recorded in the on-line system, incomplete, not paid for, destroyed or issued, acquired or presented, in, or upon, violation of these Rules. Void tickets are the property of Mackenzie Health Foundation.

4. General

  1. Mackenzie Health Foundation makes no representations of any kind whatsoever about the online system used to conduct lottery games and shall not be liable for any loss or damage suffered by any person as a result of the operation of the on-line system or the failure of the online system to operate properly or at all.
  2. Persons designated by Mackenzie Health Foundation as Related Parties are not eligible to participate in the 50/50 Lottery. Mackenzie Health Foundation designates the following persons as Related Parties:
    • Members of Mackenzie Health and Mackenzie Health Foundation’s Board of Directors; as well as their family members living in the same household
    • Mackenzie Health Foundation’s employees, including those who are on paid or unpaid leave; as well as their family members living in the same household
    • Senior Leadership Team of Mackenzie Health Foundation and Mackenize Health and independent contractors who fill these roles; as well as their family members living in the same household
    • Employees of Ascend Fundraising Solutions
    • Persons under the age of 18 years;
  3. Mackenzie Health Foundation may amend these Rules at any time and in any manner subject to the approval of the AGCO.
